You will also be required to submit the following forms with your application and payment prior to any approvals. These forms are available as a fillable PDF on the individual license page.
Payment can be made by check and included with your mailed paperwork. Or you may pay by credit card online. If paid by credit card, please include confirmation number with your mailed paperwork. There is a 2.5% convenience fee charged by the vendor.
Tennessen Warning Notice
The government must give individuals notice when collecting private or confidential information from them. This is referred to as a "Tennessen Warning Notice." Government may also call it a "Privacy Notice," a "Notice of Collection of Private/Confidential Data," or something similar. The purpose of the notice is to enable people to make informed decisions about whether to give information about themselves to the government. (See Minnesota Statutes, section 13.04, subdivision 2.) This notice will be provided anytime we collect private data and you will be required to acknowledge receiving it.
Consent for the Release of Information
This form must be completed by EACH individual applying for a permit or license requiring a background check. The consent is valid for 90 days.
Workers Compensation Compliance Certificate
Minnesota State Statute 176.182 requires evidence of compliance with workers' compensation insurance coverage. All applicants must complete this form.
Minnesota Tax Clearance Certificate
Minnesota State Statute 270C.72, Chisago County is required to provide the MN Commissioner of Revenue your business tax ID number and/or SSN for each licensed applicant.
