Chisago County Public Health’s work is guided by several department plans and data.

Community Health Assessment (CHA) 

The CHA is the result of a collaborative community effort. It is a core function of public health, as well as a state mandated responsibility to conduct a community health assessment at least every five years. The report provides the general public and policy leaders with information on the health of our county’s population.

The CHIP is a five year plan that has established goals, priorities and identified action steps to address the public health priorities

Heath Equity Data Analysis (HEDA)

The HEDA provides a framework for analyzing health disparities in communities. Advancing health equity is the attainment of the highest level of health possible for people of all ages and backgrounds in Chisago County.

Outreach and Communications Plan

Strategic outreach and communication provides a framework that ensures every staff, board member, and partner is working from the same set of assumptions and understands how their work relates to public health’s core values and goals. This plan will increase visibility and awareness of Chisago County Public Health.
